Department of Motor Vehicles:
  Must be 16 years old with proof of driver education to get a driver license. Driver transcripts (computer printouts) are $6.55 per record. Must have the full name (first, middle, last) and birth date, or the driver license number and written reason for obtaining.
 (Note: When fully implemented, the Driver Privacy Protection Act will provide access to personal information only for specified permissible purposes.) 
Payment must be in the form of a check or money order made payable to *State of Michigan*, or by credit card (Discover, MasterCard, or Visa). Mail requests to :
